Purpose
 Enrollment Form Used to enroll or re-enter the plan. Also used to designate plan investments.
 Beneficiary Form
Used to designate who will receive plan proceeds in the event of the participant’s death.

Final Distribution Forms Use to take a distribution from your 401-k plan in the event of termination of employment, retirement or disability.
Contribution Change Forms

 

Use to change the percentage of your contribute from your pay check to the 401-k plan.
Beneficiary Distribution Forms

 

Use to request a distribution to a beneficiary in the event of a death.
Rollover Contribution Forms

 

Use to rollover a qualified plan into the CWA Savings & Retirement Trust.
In Service Withdrawal Forms

 

Use to withdraw funds from the CWA 401-k plan, for reasons other than hardship or termination. Contribution suspension and penalties may apply.
Required Minimum Distribution Forms  Use to begin taking Required Minimum Distributions after age 70 1/2.
Hardship Distribution Forms

 

 

Use to withdraw funds from the 401-k plan for the purpose of a first home purchase, un-reimbursed medical expenses, post-secondary tuition expenses and related educational fees, or payments necessary to prevent mortgage foreclosure or eviction from primary residence.
Catch Up Contribution Form

 

To allow participants over the age of 50 to make supplemental contributions to the plan.
Loan Application 

 

To allow participants to apply for loans against their plan balances.
Loan ACH Payment Bank Change
To allow participants to change the bank against which their ACH loan payments are deducted.
Loan Payoff Form 
To allow participants to payoff outstanding loans to the CWA SRT before term.
